The Design and Technologies for Healthy Aging would like to invite you to the LAST meeting of the academic year. Scott Morrison, Certified Senior Advisor and President of BrightStar® will present "BrightStar Clinical Pathways: Heart Failure".
 
The meeting will be held on Thursday April 19th 9:30-11:30AM at Room 207 in the Health Systems Institute (HSI) Building, located at 828 West Peachtree Street, NW 2nd Floor Atlanta, GA 30332. Metered parking available at the address.

1 in 5 people diagnosed with chronic illness like Heart Failure is rehospitalized. BrightStar Clinical Pathways uses an RN-Coordinated, team oriented, and evidence-based approach to optimize client health and well being and reduce the likelihood of a return trip to the hospital. BrightStar Clinical Pathways is an evidence-based program inspired by national recognized care transition programs, like The Coleman Care Transitions Intervention Program that resulted in a 50% reduction in readmissions after 30 days. Patients in the Coleman clinical trial were more likely to achieve self-identified personal goals for symptom management.  There will also be discussion about the role of technology in the delivery of successful transitions.
 
Scott Morrison is the owner of BrightStar Healthcare in Duluth, GA. BrightStar is a national franchise and a premier provider of private-duty, in-home healthcare ranging from companion care to skilled nursing. In addition, BrightStar offers healthcare staffing to physician offices, hospitals, nursing homes, assisted living facilities, hospice providers and clinical studies.  Scott is Vice-President of the Forsyth County Senior Provider Network and an active member of the Pickens, Gwinnett, and Hall county provider networks. He is a member of DATHA, the NAIPC (National Aging in Place Council) and a certified trainer of the Eden at Home Initiative:  training specific to caregiving for those living with Alzheimers and dementia. Scott sits on the Healthcare Advisory Board for Forsyth county and is on the Board of Directors for the National Scleroderma Foundation, Georgia chapter. He is often consulted by medical technology companies for his expertise in the field of "Aging in Place" and has guest lectured several times at Georgia Tech.
